  1. Teacher Work Sample: Must be submitted during Seminar 3 and must be implemented during your student teaching practicum.

  2. TaskStream Directed Response Folio (Professional Electronic Portfolio): Collect artifacts and data.

  5. Lesson Plans: Written daily lesson plans are required of all student teachers. Keep in a folder. Submit to CT at least 24 hours prior to teaching lesson. Provide copies of lesson plans to CT and SS for formal evaluations.

  6. Contact Site Supervisor and Cooperating Teacher well in advance of student teaching to arrange initial meeting.

  7. Tutorial: Complete the Student Teacher Tutorial Questionnaire and submit to Field Placement Administrator prior to start of student teaching practicum.

  8. Complete evaluations of Cooperating Teacher and Site Supervisor and submit as directed (see contents sheet for deadline).

  9. Final Grade Form: Once the Site Supervisor provides a copy for your records, you will be required to post the Final Grade Form to the Assignments Write-Only newsgroup in Seminar 3. THESE GRADES WILL BE VERIFIED BY THE FIELD PLACEMENT ADMINISTRATOR.
